When someone has been admitted to hospital, the thing they look forward to the most is returning to their own home to recuperate. However, due to increased care requirements, it may no longer be possible for that person to manage at home alone.
Broomfield Care can help assist you or your loved one with temporary or long-term support to help with recuperation and getting back to regular day-to-day life.
Working closely with GPs, Nurses, Social Workers and family members, Broomfield Care can provide a smooth transition from hospital to home.
Whether a high level of care is required or simple support with the day-to-day tasks during recuperation, Broomfield Care can help.
From experience, we know that hospitals don’t always give enough notice before they discharge someone. However, there is no need to worry. If this is the case, Broomfield Care can respond quickly and start support within 48 hours of the initial enquiry.
To Make an Inquiry or Arrange an Assessment, Contact Us Via Telephone or Email
Before you commence care with us, our team will complete a no-obligation assessment with you at your home to gain a greater of what care you need, when, and why. Our team will then create a bespoke care plan that takes into consideration your needs and existing schedules so you can continue to live independently and confidently at home.
Homecare or Domiciliary care is when a person receives help from a carer within the comfort of their own home. It is provided by local authorities or a private provider.
There is a wide range of services that can be provided at home, such as personal care, specialist dementia care, and social and companionship visits.
Anyone can receive care at home, with some providers specialising in adults (18+) and others specialising in care for children. However, the most common service users are the elderly, people with disabilities, and people suffering with chronic conditions, both mental and/or physical.
At Broomfield Care, we only look after 18+ individuals.
At Broomfield Care, visits typically range from 15 minutes to an hour, depending on your level of needs and the visit type.
However, for some visits, like companionship or transport visits, we make an exception and can offer visits longer than an hour (depending on our capacity).
Personal care visits help clients, who struggle with their mental and/or physical health, to maintain their personal hygiene and keep their homes safe and clean too. Visits can be a cost-effective alternative to a care home, and mean clients can continue to live at home and have control over their existing routines.
Hospital-to-home care is often only needed temporarily, while you get back to your regular routines after a stint in hospital. The amount of time you need is down to the individual and the extent of their injury or illness.
We recognise that as you get better, your needs will change- that's why Broomfield care is there for you every step of the way, when it comes to your recuperation. Our team of expert carers can help guide you on what's best for your recovery and how to tweak your care plan as your condition improves.
Broomfield Care is a private company, meaning fees are paid for directly by clients/their families. However, some individuals may be eligible for financial support from the local council, depending on their financial position.
To find out more about our fees, you can read our brochure here.
Receiving personal care at home can be more cost-effective than moving into a care home, and means you can remain living where you love.
Currently, the average cost of a residential care home in the South West of England is £1339 a week, which rises to £1627 a week for a specialist dementia nursing home. In comparison, if you require two hours a day, at Broomfield Care, this will only cost you approximately £392 a week, or £1568 a month.
Contact us via telephone or email, to discuss your care needs with a member of our friendly team. From there, we can arrange a no-obligation assessment with you and your family to further discuss your needs, goals and any questions you may have, in person.
Our team then take the details discussed with them, and begin creating your personalised care plan.
